Medical Management Support
Our Patient Care Teams develop a plan of care designed specifically for each patient and their loved ones.
The Medical Management Support may include:
- Physician services and consultations (including home visits)
- Regular visits from a specially trained nurse
- Continuous care during crisis periods
- Short-term inpatient care
- 24-hour on-call availability
- Provision of medications related to the hospice diagnosis
- Provision of supplies and DME related to the hospice diagnosis
- Therapy services when indicated (Physical Therapy, Respiratory Therapy, Occupational Therapy, Speech Therapy)
Personal Care Assistance Support
To help alleviate family burdens and stress, we provide Hospice Aides to assist with activities of daily living.
Every day activities may include:
- Bathing
- Feeding
- Small meal preparation
- Light housekeeping
- Short breaks for caregivers
Emotional & Spiritual Support
We provide specially trained professionals to assist and support patients and their loved ones with emotional and spiritual wellbeing, such as:
- Medical social workers
- Chaplains
- Support groups
- One-on-one counseling and support services
- Respite care
- Crisis counseling
- Spiritual counseling
- Trained volunteers
- 24-hour on-call availability
