The care team

Attending Physician:

  • • Certifies hospice diagnosis
    • Approves, supports and assists in the development of the patient’s plan of care
    • Works in close coordination with the Hospice Team

Hospice Physician:

  • • Certifies hospice diagnosis
    • Coordinates pain and symptom management
    • Assists in the development of the patient’s plan of care
    • Provides in-home physician visits when needed
    • Consults with patient’s physicians regarding hospice care

Hospice Nurse/RNCM:

  • • Reviews and identifies patient’s/family’s needs
    • Assists in the development of the patient’s plan of care
    • Helps to manage medications, pain and symptoms
    • Provides support and guidance to patients and families
    • Coordinates care with Hospice Team

Hospice Aide:

  • • Provides personal and comfort care for the patient
    • Provides support to the patient, family and loved ones
    • Assists patient with daily living activity, such as bathing, feeding, light housekeeping
    • Works in close coordination with the Hospice Nurse/RN Case Manager

Hospice Social Worker:

  • • Reviews patient’s and family’s social, emotional, and financial needs
    • Assists in the development of the patient’s plan of care
    • Assists with end-of-life planning
    • Provides counseling services
    • Refers patient and loved ones to appropriate community resources

Hospice Chaplain:

  • • Non-denominational spiritual support
    • Explores spiritual concerns and needs with patient, family and loved ones
    • Facilitates life review
    • Assists in the development of the patient’s plan of care
    • Provides counseling services and bereavement support
    • Coordinates services in conjunction with patient’s personal faith leader

Volunteers:

  • • Provide companionship and support to patient, family and loved ones
    • Provide non-medical support
    • Provide short periods of respite time for the family and loved ones

Bereavement COORDINATOR/COUNSELORS:

  • • Provides pre-bereavement and grief counseling
    • Provides support group information and grief materials
    • Provides direction and insight regarding appropriate community services
